Question Answered step-by-step -An Emergency Department nurse was completing the dischargeprocess with a patient named Lucinda Ramirez. The discharge instructions included important information about how to care for her wound at home. When she was done speaking, the nurse asked Ms. Ramirez whether she understood the instructions. The patient smiled and nodded, then signed the discharge form in the place the nurse indicated. She took her copy and left the hospital, leaving the nurse with the satisfied feeling that she had done a thorough and complete job. About a week later, the Ms. Ramirez returned to the Emergency Department with her wound horribly infected. When the nurse who gave her the instructions was questioned whether or not she explained wound care to the patient she answered yes. She told her Manager that she had asked the patient if she understood; as evidence in her own defense she reported that the patient had smiled and nodded, indicating that she did understand.Question:  How do you think this scenario could have had a better outcome?
