Over the years, research has demonstrated that chronic persistent mental illness (MI) and substance use disorder (SUD) can lead to medication non-compliance and psychiatric hospital dependence. In an article written by Acar et al., (2017), several reasons led to non-adherence of treatment recommendations for those suffering from MI. Some patients may have poor insight into their MI, dislike side effects, forget to take the medication, missed appointments leading to poor patient outcomes, relapses, and hospital readmissions. With most behavioral health organizations accepting Medicare and Medicaid as payors, it is critical for facilities not to get into ethical dilemmas by admitting patients who do not meet the criteria for admission, which is Medicare/Medicaid fraud. With the demands of the heroin/fentanyl epidemic and the dually diagnosed patients, facilities are seeing an influx of the same patients. As a house supervisor at a free-standing for-profit psychiatric hospital, I have to review all referrals after-hours to determine if they are medically stable to admit to my job. There are many instances where I do not feel the referral meets the criteria for inpatient admission; in those cases, I discuss the potential patient with the clinician to understand why they felt the patient met the requirements for admission. If I feel very strongly that the referral does not meet inpatient admission requirements, I will call the administrator on call (AOC), discuss the case with them, and let them decide. If the AOC does feel the patient meets inpatient criteria, they will explain their thoughts to me, and we can agree to disagree. According to the American Nurses Association (ANA) (2015) provision 5 section 5.3, nurses are moral agents who must uphold their moral values in all situations, even when others do not approve of their professional opinion. The virus has led to specific challenges in medicine; nurses have been overworked and are in danger and doctors. Reshuffling has occurred in healthcare organizations, causing a disturbance in running the system, and funds have been utilized to combat the virus and lower its spread. Measures have been put in place to ensure that patients obtain quality service and both health workers and the patients are guaranteed safety from the virus. Furthermore, WHO has taken the step to advise governments worldwide “that they have a legal and moral responsibility to ensure health, safety and well-being of health workers” (WHO, 2021). Thus, the Coronavirus is a national threat that needs to be dealt with to ensure that people are safe from its effects, and healthcare providers can only do it           The Coronavirus as a national healthcare issue has crippled the process of developing a solution to solve the issue; this can be identified by the fact that its occurrence has led to an imbalance between the limited resources the need for quality care in healthcare facilities. In addition, engagement of healthcare facilitators keeps dropping due to the same issue, nurses are overworked, and as a result, they may quit or become less competent in their work (Talaee et al., 2020). In any case, one can deduce that Coronavirus as a national health issue hinders the development of policies that support and foster quality healthcare since it brings an imbalance in the quality of service and availability of resources in healthcare facilities.
