An issue that I anticipate within my future as a Family Nurse Practitioner is hospital readmissions within the heart failure population. The readmission of heart failure patients is high, resulting in a public health and economic burden (Su et al., 2019). Perhaps the reason for this is inadequate teaching by hospital staff. This is an area where caregivers can positively impact patient outcomes. Educating patients about their disease and the different ways they can manage their health is vital. By doing this, hospital readmissions can be greatly decreased. 2. Next, based on the problem described in question one, formulate a clinical/practice question using the “Population, Intervention, Comparison, Outcome, Time (PICOT)” format. This question will drive the literature search for your issue. In patients with heart failure, is there evidence that suggests educating the patients before discharge, compared to not educating, will improve 30-day hospital readmission? 3. Journal of Clinical Nursing (John Wiley & Sons, Inc.), 28(13/14), 2526. The focus of this study is on heart failure patients and the reasonings for their readmissions. A longitudinal observation study design was used to identify the reasoning behind heart failure readmissions. This is a great quantitative study to help aide me in the development of my PICOT question because it is saturated with information about heart failure and readmissions. My PICOT question focuses on heart failure patients and if education will help to decrease readmission rates. Ma (2019) states that there have been many therapeutic advances, in the last decade, in surgical and pharmacological treatment but readmission rates in heart failure patients are still high (p. 2527). This statement allows the reader to see why this topic needs to be further investigated. The data collected provides a great deal of knowledge as to why heart failure patients are being readmitted. This information is useful because it provides me with more knowledge to be able to answer my PICOT question. The added value of this data will help my research be more well-rounded and accurate. Based on the above information, I need help with the following questions: -Explain the purpose of the PICOT question. Discuss whether or not you can change your PICOT question once you start searching the databases using the keywords identified in it and why? -What steps can you take to ensure your PICOT question is the best it can be so you are ready to search the databases efficiently and effectively? -Please review Appendix A in your Melynk & Fineout-Overholt textbook (pp. 706-707). Next, take a moment to revise your PICOT question. -As an evidence-based practitioner, you are responsible for conducting a systematic and thorough search of nursing theories to support your studies. Using the Additional Resources found in the Learning Materials section of this model and/or independent search, select a nursing theory to support your PICOT.
