He couldn’t drive to the clinic due to the shortness of breath, so the provider told him to call an ambulance to take him to the hospital to be admitted. He wanted to have his neighbor drive him instead due to the cost. He is now being admitted directly to the Medical-Surgical Unit in moderate respiratorydistress for chronic heart failure exacerbation.  He was brought by his neighbor to the ED briefly where I was told by the nursing supervisor to meet him get his stat diagnostic testing done before bringing him up to you.Assessment: Alert and oriented, anxious, tired, and he started complaining more of shortness of breath as I was bringing him upstairs.
