Tracer methodology is used during The Joint Commission’s survey process (The Joint Commission, 2018). There are individual tracers, program-specific tracers, and system tracers, which each subset is used depending on the data being analyzed (Siewert, 2018).            Tracer methodology can be used to root out issues in inpatient care that result in pressure injuries while hospitalized. Utilizing the individual tracer, a patient’s entire experience from admission to discharge can be audited (Siewert, 2018). The patient can be selected for the tracer based on being at high risk for the development of pressure injury, or because the patient developed a pressure injury during their stay.            The individual tracer would need to look at physician orders, nursing interventions and documentation, and the administrative policies in place regarding pressure injury. For instance, was it documented that the patient was repositioned regularly, was there a physician order regarding wound care or physical therapy, and were the policies in place for reporting pressure injuries or preventing them followed?
