Hospitals have been facing higher readmissions and mortality rates related to poor discharge instructions for decades. According to Huckfeldt Dharmarajan, (2017), Escarce and Sood (2019), the most affected patients are older than 65 years and admitted with Acute MI, Heart Failure and Pneumonia. In New Jersey, Hospital X, Unit Y presents the same problem with discharge instructions in reference to the same admitting diagnoses in patients older than 65 years. Dharmarajan et al (2017) reported “readmissions have decreased since the Affordable Care Act (ACA) created Hospital Readmission Reduction Program (HRRP)”. This organization reduces payments to hospitals with high readmission rates for Acute MI, Heart Failure and Pneumonia. According to Shaw et al (2020), 30 days hospital readmissions are directly related to mortality up to 2 years. Several studies have looked at this correlation. They reported that improved inpatient care, case management and transition of care would decrease readmissions and ultimately, mortality rates related to poor discharge practices. Quality of discharge practices and patient understanding is clever to prevent readmissions and mortality. This study will look at Acute MI, Heart Failure, and Pneumonia admitting diagnoses in patients older than 65 years, and components of adequate discharge instructions.
