Question Answered step-by-step GOOD DAY I NEED SOME HELP WITH A CARE PLAN. This is 95 years old female with a history of Hypertension and osteoarthritis was brought to the ER by her grandson who stated that patient had previously admitted to the hospital due to a fall, CT scan of the pelvis showed – no fracture, dislocation or destructive bone lesions, no soft tissue hematoma identified. Patient has history of Hypertension, Asthma, Osteoarthritis of the Left knee, Prerenal Azotemia.  Admitted to Gordon ward from Ascendant Partner Ree for Long Term Care, with diagnosis of: Moderate to Severe Cognitive impairment- MMSE 10/30 -Dec 2020. Patient alert with SOB on exertion, on PRN Oxygen 2L for SPO2 less 92%, and on Salbutamol 2.5mcg every 8 hours and Beclomethasone 50 mcg inhaler twice a day. Patient requires total care with ADLS, intact skin, OT/PT/SLP/MSW assessment and Psych consult. Requires Posey vest restraints for safety.
