MP is a 47-year-old woman who has been referred to massage therapywith a diagnosis of low back pain and a physician's order to evaluate andtreat. MP complains of severe central low back pain that is aggravated byany movement, particularly forward bending. Her only current treatment is600 mg of ibuprofen, which she is taking three times a day. She reports noprevious diagnosis of back pain or other musculoskeletal conditions.MP is accompanied to clinic today by her husband. She is alert andattentive, and her overall effect is positive. MP reports no radiation of painor other symptoms into her extremities. Pain disturbs her sleep, and she isunable to work at her usual secretarial job or perform her usual householdtasks such as grocery shopping and cleaning. She reports that the painstarted about 4 days ago, when she reached to pick up a suitcase, and hasgradually decreased since its initial onset from a severity of 8, on a scale of1 to 10, to a severity of 5 or 6. She complains of no weakness, numbness,or incoordination.The objective examination is significant for restricted lumbar range ofmotion (ROM) in all planes. Forward bending is restricted to approximately20% of normal, backward bending is restricted to approximately 50% ofnormal, and side bending is restricted to approximately 30% of normal inboth directions. Palpable muscle guarding and tenderness in the lowerlumbar area occur when the patient is standing or prone. All neurologicaltesting, including straight leg raise and lower extremity sensation, strength,and reflexes, is within normal limits.
