Jordan, Minnesota, has a provider-to-patient ratio of 1:59 (Nerents, 2020). A lack of access to care creates hurdles to better health and a healthy community. Provider constraints, financial difficulty, and transportation limitations are all factors that influence access to care (Ha, 2018).  Interventions that promote in-person or remote access to health care experts can help more people get the care they need (Finney Rutten et al., 2020). Patients who have not followed up with primary care or referrals regularly, or who have a history of noncompliance with self-care, or who have had a history of illness severity may benefit from the proposed change.Resistance is a common component of the entire change process and can be manifested in a variety of ways (Jiri Mares, 2018). Furthermore, perceived threats manifest these factors.
