Shift-to-shift nursing reports, unit transfers, and transfers to other facilities are facilitated because the patient information that’s needed by receiving staff or agencies is immediately available. Because we struggle with leadership nurses continue to remain unaccountable for their responsibility with documentation within the EHR.
