Birth weight of neonate 4800 (4.8 kg; 2.18 lbs.) grams. Pre-op hematocrit (HCT) was 37%. During surgery, blood loss reported to be 1000cc and an extension of the transverse uterine incision was sutured while extending the left broad ligament. The patient is now in the recovery room at midnight and complaining of abdominal pain. BP 120/60, pulse 120. She is passing moderate amount of blood clots. Twenty units of Oxytocin in D5RL is infusing at 125cc hr. at 12:30 AM, she is passing more clots, BP 70/40, pulse 150, urine output less than 20cc since surgery. Her abdomen is distended.The physician is notified, and blood work obtained. The HGB and HCT are 9 and 32, respectively; PTT 50 seconds and fibrinogen level is 150 mg/dl. The patient is taken back to surgery. Ten minutes prior to surgery, the patient is in shock and bleeding from IV site and vagina. She makes it through surgery and hematoma removed and she continues to bleed from the left corner of uterine incision and IV site.
