Question Kitty Kraft is a 41-year-old married woman who has been in the mentalhealth unit for 3 days. She is very quiet, sits alone, and, whenever possible, remains in her room. She has downcast eyes, and her affect is without emotion. When you question her about her feelings, she states that she feels very alone and that no one really wants to be with her or speak to her. You talk with the nursing staff and realize that the staff members are reluctant to interact with Kitty Kraft because they feel helpless when they are around her. Her history is one of great sadness. Kitty Kraft has recently suffered major losses that are hard to imagine; most of the staff members are reluctant to discuss the losses with her or to interact with her. She lost her only daughter to suicide just before her admission. Her daughter was only 16 years of age. She has a remaining son who suffers from schizophrenia. Her husband rarely visits, and when he does, you do not observe any conversation between them. Nursing Assessment Kitty Kraft was admitted to the unit with a diagnosis of major depression. She has been prescribed an antidepressant medication and has an order for psychotherapy and group therapy. She keeps her individual therapy sessions but states that her problems are too different and that no one would understand; she believes it would be useless to go to group therapy. You have completed a suicide assessment, and, at this time, Kitty Kraft does not seem to be a suicidal risk. You already identified the nursing diagnosis of hopelessness, but an additional diagnosis seems to be associated with Kitty Kraft’s withdrawal and desire to isolate herself.
