Robbs, MDThis is a female, gestational age 39 weeks, 3 days, born by Cesarean section in this facility, 11/01/21, 05:45. The patient has bilious vomiting and a heart murmur. This is my first time seeing the patient in the hospital.EXAM: VS T 37.1 (ax), P 150, R 45, BP 75/50, oxygen saturation 99% in room air. Height, weight, and head circumference are at the 50th percentile. She appears jaundiced and has a flat facial profile. Her heart is has a murmur. A nasogastric tube is placed and IV fluids are administered. An echocardiogram demonstrates heart murmur, which is medically managed. A chromosomal abnormality is suspected and a karyotype is done. Trisomy 21 is diagnosed.DX: Trisomy 21 (Down syndrome) and Neonatal jaundiceRECOMMENDATIONS: Newborn has Neonatal jaundice and Trisomy 21.
