Instead of informing my shadow nurse and instructor about the oxygen saturation and blood pressure, I kept on helping her with the bedpan for urination. Here, my clinical judgment was wrong, and I prioritized basic care rather than breathing and vital signs. I also documented the same oxygen saturation and blood pressure without doing any intervention and informing the assigned nurse as well. After few minutes, the shadow nurse re-assesses the patient and started yelling at me for recording oxygen saturation (SPO2) at 86% without any management. With the help of my clinical instructor, I took oxygen saturation and blood pressure again and was recorded 86% at room air and 100/90 mm of Hg respectively. After seeing the doctor’s order, I kept the patient on Oxygen with nasal prong @ 2L and her blood pressure was always in the same range since admission. I monitor it every half an hour for 2 hours. Then, the Patient oxygen saturation (SPO2) level was maintained at 92% in 2L via nasal prong. With the help of my instructor, I documented all the incident reports.
