Mr. Count came in with nausea and vomiting and dehydration Mr. Count has a history of colon cancer and had been getting chemo, but he has not been able to hold anything down for the past three days. He was given Zofran 30 minutes ago. Today his vital signs show that he has a pulse of 114. The pulse feels weak but is regular. His heart is beating faster than normal but this could be due to the patient being dehydrated. His temperature was 98.6 °F (oral). The patient has diarrhea and explains having six or seven bowel movements. Has vomited two to three times for the past 3 days. There’s a gurgling sound in the abdomen and the patient has pain on the LL side of the abdomen and masses on the LLQ. The patient is feeling numbness on the lower extremities but there’s no edema and the circulation on his feet is good. My assessment is that he is dehydrated due to diarrhea and vomiting My recommendation is to continue with the IV fluid for hydration and a complete blood cell count to identify anemia or infection. I also recommend testing the patient’s stool for C. difficile.
