Jairaj says that Prisha frequently leans on him or the wall for balance when mobilising around their home and that he is worried she may fall and hurt herself. RN James jots this information on a piece of paper meaning to document it in Prisha’s chart before she is transferred to the ward. RN James forgets to document the conversation and does not hand this information over to the ward nurse accepting Prisha. Jairaj, thinking that the ED nurse has passed on the information he told her, does not mention anything about Prisha’s cognitive state or balance problems. On the evening shift, Prisha gets up to go to the toilet, loses her balance and falls fracturing her right arm. Prisha called out in pain and a RN Hardy, who was in the next room, raced in to find her lying on the floor. RN Hardy immediately called for help, placed a pillow under Prisha’s head and sat on the ground supporting her broken arm, while calmly reassuring Prisha. After helping Prisha back to her bed, Prisha was reviewed by the on-call doctor who immediately called the orthopaedic surgeon. After checking Prisha’s injury, the surgeon decided to take her straight to the operating theatre to repair the badly fractured arm. The surgeon got a consent form and asked Prisha to sign it, which she did. After the surgeon left, RN Hardy started to prepare Prisha for surgery when Prisha said, ‘what are you doing, don’t touch me’. RN Hardy explained that she was going to have her arm fixed in the operating theatre and Prisha replied, ‘I’m not having an operation, get away from me’. RN Hardy immediately stopped what she was doing and called the surgeon because she felt it was not in Prisha’s best interest for her to explain the procedure or to continue without Prisha’s cooperation.
