Nil signs of pain. CVS.Vitally stable. Afebrile. RESP.RR17. Spo2 99% on RA. Nil SOB. Nil increase WOB. Chest clear.GIT.Pureed diet and mod thick fluids. Tolerating good amounts of oral intake as per food record chart. He is continent. BO in pan this mane – as documented on the bowel chart. He has his medication crushed with meals.
