Patient has history of Hypertension, Asthma, Osteoarthritis of the Left knee, Prerenal Azotemia.  Admitted to Gordon ward from Ascendant Partner Ree for Long Term Care, with diagnosis of: Moderate to Severe Cognitive impairment- MMSE 10/30 -Dec 2020. Patient alert with SOB on exertion, on PRN Oxygen 2L for SPO2 less 92%, and on Salbutamol 2.5mcg every 8 hours and Beclomethasone 50 mcg inhaler twice a day. Patient requires total care with ADLS, intact skin, OT/PT/SLP/MSW assessment and Psych consult. Requires Posey vest restraints for safety.
