Question Answered step-by-step Could I get some assistance with this medical coding question for… Could I get some assistance with this medical coding question for ICD PCS I have pasted the scenario – I just need help with finding the answer PREOPERATIVE DIAGNOSIS: Acute appendicitis POSTOPERATIVE DIAGNOSIS: Acute appendicitis with perforation PROCEDURE PERFORMED: Open Appendectomy INDICATIONS: This is a 38-year old male patient seen today for suspected acute appendicitis and is taken into surgery.OPERATIVE REPORT: The patient was brought to the OR suite, draped, prepped and induced with general anesthesia. He was placed in the supine position, exposing the right abdomen. A transverse incision was made in the subcutaneous tissue and dissecting it down so that the external fascia could be opened to expose the lateral edge and lateral half of the rectus muscle, which was divided with electrocautery, as was the internal oblique musculature. Purulent fluid was recovered upon entering the peritoneal cavity.The tip of the appendix was then brought up into the incision and it could be seen that it was grossly inflamed with fibrinous material. The perforation was in the proximal third of the appendix. Part of the mesoappendix was clamped and cut and then the proximal end tied with #2-0 chromic. This freed the appendix up, along with the necrotic proximal portion. The appendix was amputated between the tie and the clamp. An additional #3-0 silk figure-of-eight was used to imbricate the stump. Once this was done, the right pelvis was examined and found to be free of other debris. Irrigation returned no foreign material. The deep fascia along the transversalis and the peritoneum were closed using a single strand #0 Vicryl. The internal oblique musculature and superficial fascia was reapproximated using additional #0 PDS..
