Pain is felt beneath the clavicle and in the neck and left scapular region. The pain was worse when lying flat, breathing, twisting body and relieved by leaning forward. He had no sweating nor fever. His history includes recent infection with Coxsackie B Virus which is treated at home. He had a 40 pack year smoking history and a significant familial premature coronary disease. His vaccination schedule was up to date, and he had not travelled recently. On examination his temperature was 36.9°C, blood pressure was 120/83 mm Hg, heart rate was 86 beats/min, respiratory rate was 16 breaths/min and an O2 saturation of 99% on ambient air.
