A CT head was  performed and revealed a large right frontal intraparenchymal hemorrhage. TheEmergency MD then spoke to the neurosurgeon and the ER was prepared for the arrival of the patient. After few hours of observation @ER, the patient’s condition deteriorates, the following was noted GCS= 4, with on and off generalized seizure, pupils anisocoria, vital signs BP= 130/90, HR=145, RR= 30, with signs of increased ICP, MAP=50 The neurosurgeon team decided to have the patient for craniotomy in the afternoon. After OR, now on SICU, the patient now is intubated with the settings of  TV=100%,FI02=100%, RR=20.
