Patients are going to getting prescriptions from different providers at different facilities that are not aware of the patients past medical history. The following database proposal is designed to facilitate physicians when prescribing narcotics to patients to eliminate overdose situations and decrease substance abuse from prescription narcotics.Database Goal and Objectives Physicians need a database that will generate a patient’s past medication history as well as any substance abuse treatments in a simple to access and easy to use format. The proposed Narcotic Database will provide doctors with patient information regarding past prescriptions and consumption of narcotics as well as if the patient has been seen and/or treated for substance abuse. The proposed database will attempt to capture the clinical data that is related to pharmaceutical products and utilization. The goal of the final database is to identify a correlation between narcotic addiction rates and the original year that the patient consumed a narcotic.Purpose of Database Schema           Database schemas are a visual overview of the information that will be contained in the database. They help to identify the different tables used for a database and the fields that are in each table. Database schemas also describe the relationship between the different tables in a database (Entity Relationship Diagram (ERD), n.d.). The purpose of the Narcotic Database Schema is to illustrate the relationships between the patient, patient records, hospital pharmacy, the treating physician, and the substance abuse treatment program (SATP). The database schema also provides the attributes associated with each of the above entities.Process Associated with Business Rules Relating to the Database           Business rules are “a description of a policy, procedure, or principle within an organization” (Coronel & Morris, 2017). It is important to identify and document the business rules with designing a database. These rules allow the database creator to develop the relationship participation rules and constraints and to Make a correct data model. It is important for a database creator to know and understand the business processes, and the nature, role and scope of the data in order to Make a database that will be useful to the organization. For the Narcotic Database, the following business rules apply.1.    A patient may generate many patient records.2.    A patient record is generated by only one patient.3.    A doctor may write many medication prescriptions4.    A SATP may have many patients5.    A patient may be admitted to many SATPs.Data Dictionary and Entity-Relationship Diagram           A data dictionary is used to list and define key terms utilized by an organization. It is important for employees to use the same terminology when referencing different elements of a database so that there is no confusion between similar fields (Butlion, 2020). The data dictionary and the entity-relationship diagram for the Narcotic Database is attached in the assignment submission.Difficult Areas to Develop and Assessment of Difficulty Encountered in Modeling           One area of this database that was especially difficult to develop was the SATP entity and the associated attributes. This area presented a challenge due to the lack of personal knowledge of this facilities workflow and documentation process. Interviewing employees at a SATP would provide additional information and facilitate in creating a database that the facility would utilize.Project Limitations and Possible Extensions           The limitations of this database are that patients that have a substance abuse problem will go to different facilities to get prescriptions. Patients have also been known to use different identification in order to obtain narcotics. If the database does not include the different facilities the patient visits or the different patient identifications that the patient uses, their medication history will not be available to the current prescribing physician. This database has several possible extensions that include the addition of different healthcare facilities and multiple substance abuse treatment facilities.
