Question Answered step-by-step CASE STUDIES: John Brown is a 58 year old man. He had a stroke 18 months ago and as a result of the stroke John has a left sided hemiplegia. He also has expressive dysphasia and dysphagia. The expressive dysphasia makes it difficult for John to communicate. The dysphagia affects John's swallowing and he is on a soft diet with thickened fluids.John is dependent upon support workers for all activities of daily living (ADL'S) . He requires full assistance of two care workers and he uses a hoist for all transfers. John has been drinking a lot of sweet smoothies lately and it has been observed that he has gained weight. This is concerning due to the fact John has heart disease and high blood pressure.John does have urinary incontinence and at this time is taking antibiotics for a urinary tract infection.John had always been an active and social man, he loved playing darts and fishing. As a result of the stroke, John now has depression. You are visiting John today to support him with his ADLs. It is a hot day outside but John's house is well air-conditioned. You notice he is flushed and sweaty, and when attending to his personal care you observe his urine to be dark and concentrated. John says he has a headache.
