The company I currently am employed with still has the nurses utilizing paper charting. The company provides care for clients who have worked at the government plants in Oak Ridge, TN. The DOL guidelines only allow charting on these clients covered (by DOL) conditions. This is hard for a lot of nurses who have always been taught to document everything and cover yourself. So, there are a lot of corrections that need made by the nurses every week. If the company used electronic charting, I think it would be easier. Case managers do have an app called Perfect Serve that we use. This app is downloaded onto our phones and allows us to communicate with doctors, other nurses, managers, etc. We also have access to all available client information. We can send and receive images, etc. So, it comes in handy when we are a long way from the office, and someone needs something sent immediately. Interoperability within healthcare (nursing) allows for various information technology systems to interact with one another and exchange data that is important and usable. Functional (foundational) interoperability is the lowest level. Like a client portal. This is data going from one system to another without expectation of interpretation. This is the most basic of the tier of interoperability. Structural interoperability is next level in the tier. This level requires data follow a structure / format (uniform movement). Data is stored within the receiving system. This data will be preserved and unaltered (in its original form). An example could be data from connected devices (like vital sign machines, heart monitors). To achieve structural interoperability, the recipient should be able to interpret information. Semantic interoperability is the highest level. Within this level, data is sent, received, interpreted, and used to achieve an outcome (like improved client care and safety), and stored and preserved. Case managers could use this type to base needed interventions from something like a high risk fall assessment score. A complete medical record is necessary for continuity of care.
