Kites’ grades are declining, and she has gotten into fights which is new behavior. Kites had always been a really good student. Her parents bring her to the pediatrician to discuss their concerns.CHIEF CONCERN:”I don’t know what to do with what Kites is telling me. Is this normal growing pains of puberty?”HISTORY OF CHIEF CONCERN:Kites’ sex assigned at birth was female. Kites wears binding sports bras and oversized clothing. kites has also expressed an attraction to girls. Kites reports teasing by other kids at school. Susan ‘s best friend is gay, but Marvyn has no one he is close to that is openly gay or transgender. Susan wonders if Kites is gay.FAMILY PROFILE:Kites’ family structure includes two parents, Marvyn and Susan , and a younger brother, Renz. They live in a single-family home, and both parents work Monday through Friday schedules. They are “comfortable” financially. Both children go to school and attend an after-school program. They have extended family in town who they visit frequently. Although the family doesn’t participate in religious activities regularly, Marvyn had a strict religious upbringing, which affects how he feels about social issues.PAST HEALTH HISTORY:Kites is of normal weight and has always been healthy and achieved good grades. Recently, Kites started to restrict during meals in an attempt to lose weight. She had always been gregarious and made friends easily. In the past few months, however, she has stopped sharing as much with Marvyn and Susan and has stopped hanging out with a good friend, Raissa. Kites spends most days alone after getting home from school.FAMILY HEALTH HISTORY:Marvyn is healthy with some mild hypertension controlled by medications. Susan is also healthy. She has an active social life and exercises regularly.
