Question Answered step-by-step 43 y/o F established pt with PMHx of migraines , presents to the clinicto obtain annual examination . Pt c/o of SOB, palpitation and fatigue for the past 3 months but it has been getting worse. Patient also complains of hair loss that has improve taking over the counter Biotin. Patient is under a lot of stress due to problems at her job. She is divorced and lives with her 2 young sons ages 10 and 9. Patient doesn’t take medications on a regular basis and denies any previous medical condition. Her LMP was 2 weeks ago and she is taking OCP. Denies recently traveling and she doesn’t smoke, drinks alcohol or use any recreational drugs. ital Signs HR 126RR 18BP 140/86BMI 23 T-98.4  Upon Physical Examination  – Grade goiter was presented. Moist skin, hand tremor, and tachycardia (126/min) were presented as well. Lungs clear with no adventitious sounds.
