Her pain score is a 5/10 at the incision site. Hannah states that a pain score of 3/10 would be tolerable for her.  The nurse performs her BUBBLE assessment and notes breasts are normal, uterus is boggy, midline, and U/U with moderate bleeding that continues to trickle. 2.What is your priority at this time? After interventions the bleeding has returned to normal.
