To complete Assignment 5: Survey Questionnaire Design, please follow the prompts below to construct a
survey questionnaire to submit for this assignment.
1. The purpose of this survey instrument is to collect data about the public’s understanding of and opinion
on climate change. The data will be used to evaluate how the public’s understanding and opinion are
affected by their socio-economic (income, race, occupation, etc.), demographic (age, gender, place, etc.),
and educational background.
2. You will need to design a questionnaire about climate change to find out how much people know about
this subject based on the criteria in step 1 above.
3. The questionnaire should include at least 10 questions, which cover the scientific basis, evidence,
mitigating policies and solutions, and personal opinions of climate change.
4. One or two questions should be open-ended; the other questions should be closed-ended.
5. The closed-ended questions should have at least 3 different types (Likert rating scales, forced-choice
alternatives; ranking of alternatives, adjective or activity checklist; paired or triadic comparisons; sorting
task, etc.).
6. Remember, when preparing a survey instrument, you also have to think about coding the items for
subsequent analysis. Along with preparing this survey, you should also prepare a separate coding
document to describe how you plan to code each item on the survey.
