Thick, yellow drainage from wound noted. Right heel has reddened area that does not blanch. Heart tones are regular. Lung sounds clear bilaterally. Abdomen soft with hypoactive bowel sounds. Pedal pulse is palpable and regular. Capillary refill is less than 2 seconds in all extremities. Client is passing gas but has not had a bowel movement today.
