On Friday the schedule was fully booked for the dentist, Dr. Jackson (DDS) and the dental hygienist, Karen (RDH, with one year experience and Louise’s friend). In addition, the office is short staffed because one of the dental assistants called on sick leaving Louise as the only dental assistant working on that day.  Dr. Jackson and Karen (RDH) are running behind schedule all day due the shortage of assistant and the need of extra time to do sterilization duty. Moreover Karen’s first client was 15 minutes late to her appointment and had not had a chance to help Louise to clean and sterilize instruments from the morning clients. Karen sterilizes a couple of scaling kit instruments for herself and louise knowing that she is already overloaded with work and is busy with Dr. Jackson. When Karen (RDH) grabs the instruments, she notes that the scaling kits are not sterilized as she mistakenly overpacked the instruments on the autoclave but since her last client of the day is waiting since 30 minutes past his appointment for debridement Karen needs to take a decision between to use them on the client or not.  Also, she got in an uncomfortable position because she doesn’t want to put her job at stake as it was her who commit the mistake of over-packing the autoclave.
