Apply system development life cycle for Health IT to an ambulatory… Apply system development life cycle for Health IT to an ambulatory EHR case study.Dr. Jones was a full-time faculty physician in infectious disease and his employer was a multihospital system in a major metropolitan area. The system operated a methadone maintenance and treatment program and this program was awarded a grant to provide primary human immunodeficiency virus (HIV) care to former substance users in the inner city. This was a public-private initiative to improve healthcare for the underserved HIV population within the city. Dr. Jones became the Medical Director for this grant program and the requirements of the program included collecting and reporting grant-required specifications. The program provided a standard of care reflecting the best evidence-based medicine at the time: HIV monitoring (CD4 counts), antiretroviral therapy, prophylaxis to prevent opportunities infection, screening and treatment for tuberculosis, screening for Chlamydia and gonorrhea, periodic pap smears, and mental health assessment were amount the key standards tracked for this grant. Initially the program utilized paper forms but the amount of data became overwhelming and so did the amount of lost or missing paper forms. Dr. Jones felt that an EHR could more easily capture the required data and that missing paper charts would be easily eliminated by purchasing an EHR.The healthcare organization at the time had no formal software purchasing process so with the Quality Committee’s approval, Dr. Jones sought hardware and software for an EHR with a grant refunding application. Once funding was approved, Dr. Jones selected one of the better known EHR products. As time drew near to purchase the EHR, Dr. Jones felt confident that the project would be a complete success. The clinical team wanted the EHR and the grant sponsor approved the budget modification for purchase of an EHR. What could go wrong?Dr. Jones was offered and took another job opportunity with the healthcare system. Dr. Jones could no longer work on the EHR project but he had a colleague (Dr. Smith) at the Clinic who was very technical so he felt that Dr. Smith would be a good person to carry on the baton. Unfortunately Dr. Smith technically knew about EHRs but his change agent skills were lacking. Several months later, Dr. Jones stopped by the HIV Clinic to see how the EHR was working and he was surprised to find that that the same paper-based charts were in place and the EHR had not been implemented.
