The client was dressed well, alert, and orientated to person, place, and time but could not recover the data. Also, she complained that she usually forgets her daily routines meeting time, and her school kids have to remind her of assignments each time before leaving class Background: The client teaches an undergraduate student with no known allergy and no social history of smoking and alcohol. The Mental Status Exam, Mrs. Baker, describes forgetfulness with her schedule, assignments for her students, and days and times. The client describes in detail what she does. However, Mrs. Baker is knowledgeable and mindful. Action: Mrs. Baker accepts that her memory has become substandard, and she is looking for answers and help. Mrs. Baker admitted that she had gotten in her car and drove off, forgetting where she was, which seemed to disappoint her a lot. Also.
