Schedule a discussion with your two (2) colleagues who responded to the questionnaire. Ensure that you have secured necessary permissions from your supervisor and colleagues prior to commencing this task. 2. In your discussion, clarify with your colleagues your work role and the purpose of the meeting. Inform them that the data gathered relating to them is for assessment purposes only. Ensure them that you will maintain the privacy and confidentiality of their information. 3. Discuss with your colleagues the results of the questionnaire based on your analysis. Complete the Action Plan template provided in this workbook. 4. In your Action Plan, identify at least two (2) current areas of need in the workplace, and two (2) potential areas of need, in line with minimising or eliminating the stress experienced by the workers. 5. Discuss and identify strategies on how each area of need can be addressed. Specify the following: ? Purpose of the proposed action plan ? Person/s responsible for the strategy ? Expected outcomes for each strategy ? Expected completion date of the action plan Guidance: This is not required for the potential areas of need. ? Monitoring and review procedure 6. Sign the completed Action Plan template and have your supervisor/relevant personnel sign it. 7. Provide each of your colleagues a copy of the Performance Standards Checklist. 8. With your colleagues, identify two (2) professional performance standards/key performance indicators from their role. Have them document it in the Professional Performance Standards column of the template. Guidance: Include only those which are regularly monitored and can be measured during your vocational placement. These can include timeliness of reports, working with care plans, participating in case conferences and assessments, etc. 9. Identify two (2) personal performance standards from each worker. Have them document it in the Personal Performance Standards column of the template. Guidance: Ask each worker the standards they personally set for themselves in terms of completing their responsibilities in the organisation, e.g. being open to feedback of colleague or supervisor, shows flexibility to roles and tasks assigned, etc. 10. Ask your colleagues to assist you in monitoring their current health and well-being by providing a brief reflection on how they have performed their tasks, according to the standards they have specified. 11. The checklist must be completed for at least two (2) days when they have worked. Have your supervisor/relevant personnel sign off the checklist to ensure validity and authenticity of the participants.
