The patient was brought for consultation due to body weakness, mild fever, anorexia, and bloody-mucoid stool. During the interview, the patient’s parents revealed that the dysentery is lasting for about 4 days now and is accompanied by vague abdominal pain, constipation, and intense desire to defecate (tenesmus). When the patient was interviewed, he told the nurse that prior the symptoms, they consumed some street foods and juice in a nearby market after school. The patient was then referred to the nearest hospital.  In the emergency department, palpation of the abdomen showed hepatomegaly and mild abdominal distension. Skin on the abdomen recoils after 4 seconds of pinching and the patient’s mouth and lips are apparently dry. The admitting physician ordered stool examination which tested positive for entamoeba histolytica cysts. Complete Blood Count Result are as follows: Hct 60%, Hgb 11 mg/dL, RBC 5000/mm3, WBC 14000/mm3. Serum potassium is at 3.4 mEq/L and serum Sodium is 132 mEq/L.Admitting orders are as follows:1. Admit patient to room of choice under the service of Dr. Paraluman (gastroenterologist)2. Hook PNSS 1L x 8 hours3. Start metronidazole 500 mg TIV q8 hrs for 7 days4. Kalium Durule 1 durule BID for 3 days5. Repeat serum sodium and potassium after 3 days6. Paracetamol 300mg TIV for fever ? 37.8 degrees Celsius7. Encourage to increase oral fluid intake8. Soft diet9. Monitor I and O q shift10. VS q 4 hrs11.
